Eighteen assisted living communities managed by Gardant have been recognized as 2024 recipients of the Bronze – Commitment to Quality Award by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL) for their commitment to improving the lives of residents through quality care. The distinction is the first of three progressive award levels through the AHCA/NCAL National Quality Award Program. The program, presented by the leading association in long term and post-acute care, honors providers across the country that have demonstrated their commitment to improving quality of care for our nation’s seniors and people with disabilities.
The Gardant-managed communities to be honored with 2024 Bronze Awards are as follows:
Brookstone Estates of Effingham
Brookstone Estates of Effingham is an affordable assisted living community located in Effingham, Illinois. It has a total of 46 private apartments. The community is designed to serve low-income adults 65 years of age and older.
Brookstone Estates of Fairfield
Located in Fairfield, Illinois, Brookstone Estates is an affordable assisted living community designed to serve adults 65 years of age and older. Brookstone Estates houses a total of 46 private apartments.
Brookstone Estates of Harrisburg
Featuring 46 private apartments, Brookstone Estates of Harrisburg, Illinois is an affordable assisted living community and designed to serve adults 65 years of age and older.
Brookstone Estates of Paris is located in Paris, Illinois. The affordable assisted living community is designed to serve adults 65 years of age and older. It has a total of 46 studio, one-bedroom, and two-bedroom apartments.
Brookstone Estates of Robinson
Brookstone Estates of Robinson is an affordable assisted living community located in Robinson, Illinois. With 42 private apartments, the community is designed to serve adults 65 years of age and older.
Located in Tuscola, Illinois, Brookstone Estates of Tuscola is an affordable assisted living community that serves adults 65 years of age and older. It features 46 private apartments.
Brookstone Estates of Vandalia
Located in Vandalia, Illinois, Brookstone Estates of Vandalia is designed to serve adults 65 years and older who need some help to maintain their independence. Brookstone Estates of Vandalia features a total of 46 private apartments.
Carriage Court of Washington Court House
Located in Washington Court House, Ohio, Carriage Court is designed to serve adults 65 years of age and older. They have a total of 47 studio apartments.
Evergreen Village at Fort Wayne
Evergreen Village at Fort Wayne opened in the summer of 2020 and is especially designed to serve the older adult. This three-story building features studio and one-bedroom apartments, and the community offers a full spectrum of services that help seniors embrace their independence.
Grand Victorian of Rockford is in Rockford, Illinois and is designed to service adults 55 years of age and older. It offers a total of 62 private apartments.
Located in DeKalb County, Illinois, Grand Victorian of Sycamore is an assisted living community serves those 55 years of age and older. Grand Victorian of Sycamore features a total of 61 private apartments.
Glasswater Creek of Plainfield
The community, which is designed to serve adults 62 and older of all incomes who need some help to maintain their independence, is in Indianapolis, Indiana. Glasswater Creek of Plainfield has a total of 131 private apartments.
Heritage Woods of Bolingbrook is nestled on the south side of Boughton Road between Interstate 355 and Rte. 53. Heritage Woods is designed to serve adults 65 and older of all incomes who may need some assistance to maintain their independence.Heritage Woods of Bolingbrook houses 105 private studio and one-bedroom apartments.
Heritage Woods of Dwight is located off Interstate 55 in Livingston County, 70 miles southwest of Chicago, between Joliet and Pontiac. The two-story community houses 68 studio and one-bedroom apartments, designed for adults 65 and older.
Heritage Woods of Mt. Vernon is located on south 42nd Street, east of I-57 and south of Highway 15 in the south-central Illinois community of Mt. Vernon. It houses 66 private studio and one-bedroom apartments designed to serve adults 65 and older.
Heritage Woods of South Elgin is located on the southwest corner of McLean Blvd. and Bowes Road, between Hwy. 31 and Randall Road in South Elgin. The assisted lifestyle community opened in the summer of 2009. Heritage Woods of South Elgin has 150 private apartments for adults 65 years and older who need some help to maintain their independence.
Lacey Creek Supportive Living, which opened in the fall of 2016, serves older adults of all incomes who need some help to maintain their independence. The three-story building is 108,000 square feet and houses 120 private studio and one-bedroom apartments.
The community, which is designed to serve adults 65 and older who need some help to maintain their independence, is located just east of Colorado and Philo, near the Stone Creek Golf Course in Urbana. Prairie Winds of Urbana has 94 studio and one bedroom apartments.
In the past four years, 52 assisted living and memory care communities managed by Gardant have been recognized with Bronze awards.

Created by AHCA/NCAL in 1996, the National Quality Award Program is a rigorous three-level process that is reviewed and judged by trained experts against a set of nationally recognized standards for organizational excellence. The standards of the Baldrige Performance Excellence Program help organizations achieve superior performance to improve quality of life and care of long term care residents and staff.
Providers begin the quality improvement process at the Bronze level, where they develop an organizational profile with essential performance elements such as vision, mission statement, and key strengths and challenges. Bronze applicants must also demonstrate their ability to implement a sustainable performance improvement system. Trained examiners review each application to determine if the center has met the demands of the criteria. As recipients of the Bronze – Commitment to Quality award, these communities may now move forward in developing approaches and achieving advanced levels of performance that meet the Silver – Achievement in Quality award criteria.
