Are you 65 or older or an adult 22 to 64 years of age with a physical disability?
Are you in need of some help to maintain your independence?
If you are a resident of Illinois, you may qualify for a special program called Supportive Living.
The program enables you to . . .
The program provides a great alternative to nursing home care or to struggling alone at home. It is open to individuals of all financial means, including those who are on or who would qualify for Medicaid.
Gardant Management Solutions is the largest operator of Supportive Living communities in Illinois. Our focus is on providing residents with the Love, Compassion and Dignity they deserve in addition to the help and assistance they need.
Communities that we manage are located throughout northern, central and southern Illinois, including in the City of Chicago, surrounding suburbs and the St. Louis Metro East area.
Two of the communities – Eagle’s View in Champaign County and Heritage Woods of South Elgin – have specialized neighborhoods of apartments for older adults with Alzheimer’s disease or related dementia.
We are getting ready to open two new communities this fall. Gateway at River City in Peoria will serve adults with physical disabilities. We operate a similar community in Huntley, Illinois. Lacey Creek is in the final stages of construction in Downers Grove. It will serve adults 65 and older.
In the spring, we expect to open the Lawndale Supportive Living community, located at 18th and Kostner in Chicago and Heritage Woods of Minooka, located southwest of Joliet. Both will serve adults 65 and older.
Information about the Supportive Living communities that we operate can be found on our website. Or call us toll-free at 1-877-882-1495.
